    Complete this form and submit it to the Director of Student Life after the Organization/Club Advisor has approved and signed it. Upon receipt of the request, the Director of Student Life will consult the calendar and if there is no major conflict approve the event and notify the Club Advisor and/or Club Chair. NB: Events/activities are scheduled on a first come first served basis. To ensure that the date you want is available, submit this request as soon as possible.
